โครงการพัฒนาต้นแบบของเทคโนโลยีที่ช่วยในการเชื่อมโยงข้อมูลสุขภาพของผู้ทำการในอากาศ
โดยผ่านเครือข่ายดิจิทัลความเร็วสูง
ข้อตกลงเพื่อยืนยันตัวบุคคลก่อนการใช้บริการต้นแบบของเทคโนโลยีที่ช่วยในการเชื่อมโยงข้อมูลสุขภาพของผู้ทำการในอากาศโดยผ่านเครือข่ายดิจิทัลความเร็วสูง
“AirMed” คือต้นแบบของเทคโนโลยีที่ช่วยในการเชื่อมโยงข้อมูลสุขภาพของผู้ใช้บริการโดยผ่านเครือข่ายดิจิทัลความเร็วสูง
ประชาชนและผู้ทำการในอากาศสามารถเข้าถึงต้นแบบของเทคโนโลยีที่ช่วยในการเชื่อมโยงข้อมูลสุขภาพโดยผ่านเครือข่ายดิจิทัลความเร็วสูงกล่าวคือ ผู้ใช้งานสามารถลงชื่อเข้าใช้งานระบบ (Log in) เพียงครั้งเดียวผ่านบริการยืนยันตัวบุคคล (e-Authentication Service) ก็สามารถเข้าใช้งานระบบต้นแบบของเทคโนโลยีที่ช่วยในการเชื่อมโยงข้อมูลสุขภาพของประชาชน รวมถึงผู้ทำการในอากาศโดยผ่านเครือข่ายดิจิทัลความเร็วสูงและเพื่อปรึกษาพยาบาลและแพทย์ได้หลาย ๆ ระบบโดยไม่ต้องลงชื่อเข้าใช้งานซ้ำอีก ซึ่งบริการดังกล่าวจะมีการควบคุมและรักษาความปลอดภัยด้วยมาตรการที่เหมาะสมตามที่หน่วยงานเวชศาสตร์การบินและมหาวิทยาลัยนเรศวรได้กำหนดร่วมกันไว้แล้วหรือที่จะกำหนดขึ้นในอนาคต
ซึ่งในข้อตกลงการให้บริการนี้ ให้
“ผู้ให้บริการ” หมายความถึง สถาบันเวชศาสตร์การบินกองทัพอากาศ กับโรงพยาบาลในเครือข่าย
“ผู้ใช้บริการ” หมายความถึง ผู้สมัครใช้บริการในโครงการ
ผู้ให้บริการและผู้ใช้บริการได้ตกลงกันโดยมีข้อความดังต่อไปนี้
- ผู้ใช้บริการจะต้องสมัครใช้บริการหรือลงทะเบียนผ่านช่องทางต่าง ๆ ที่ผู้ให้บริการกำหนด โดยการสมัครใช้บริการจะไม่เสียค่าใช้จ่าย ทั้งนี้ ผู้ใช้บริการสามารถใช้บริการได้ภายหลังจากได้มีการรับลงทะเบียนเรียบร้อยแล้ว
- ผู้ใช้บริการจะต้องให้ข้อมูลส่วนตัวหรือรายละเอียดที่เกี่ยวข้องกับผู้ใช้บริการตามความเป็นจริง เพื่อประโยชน์แก่การใช้บริการ หากผู้ให้บริการตรวจพบว่าข้อมูลส่วนตัวหรือรายละเอียดที่เกี่ยวข้องกับผู้ใช้บริการไม่ชัดเจน ไม่ถูกต้อง หรือเป็นข้อมูลเท็จ ไม่ว่าผู้ใช้บริการจะให้ข้อมูลนั้นด้วยความสมัครใจหรือความประมาทเลินเล่อก็ตาม ผู้ให้บริการมีสิทธิที่จะยกเลิกบัญชีของผู้ใช้บริการโดยมิพักต้องบอกกล่าวล่วงหน้า
- ในการใช้บริการบางกรณี ผู้ใช้บริการจะต้องทำการยืนยันตัวบุคคลโดยใช้ e-Mail หรือหมายเลขบัตรประจำตัวประชาชน 13 หลัก ตามที่ผู้ให้บริการกำหนด
- ห้ามมิให้แอบอ้างหรือกระทำการใด ๆ อันเป็นการละเมิดสิทธิส่วนบุคคล โดยการใช้ข้อมูลของผู้อื่นในการสมัครใช้บริการหรือลงทะเบียนผ่านช่องทางต่าง ๆ ซึ่งผู้ที่กระทำการฝ่าฝืนดังกล่าว ต้องรับโทษตามที่กฎหมายกำหนด
- ผู้ใช้บริการจะต้องยินยอมให้ผู้ให้บริการใช้ข้อมูลจากการสมัครใช้บริการหรือลงทะเบียนผ่านช่องทางต่าง ๆ เพื่อให้ผู้ใช้บริการสามารถเข้าถึงบริการได้
- ผู้ให้บริการจะเก็บรักษาข้อมูลของผู้ใช้บริการไว้เป็นความลับ โดยจะไม่นำข้อมูลดังกล่าวไปเปิดเผย นำไปใช้ประโยชน์ทางการค้า หรือกระทำการอื่นใด นอกจากการดำเนินการทางกฎหมายหรือผู้ใช้บริการให้ความยินยอม
- ผู้ใช้บริการจะต้องรักษาชื่อบัญชีผู้ใช้งานและรหัสผ่านไว้เป็นความลับ หากปรากฏว่ามีผู้อื่นสามารถใช้งานผ่านบัญชีของผู้ใช้บริการได้ ผู้ให้บริการจะไม่รับผิดชอบต่อความเสียหายที่เกิดขึ้น
- ผู้ใช้บริการจะต้องปฏิบัติตามเงื่อนไขภายใต้ข้อตกลงการให้บริการโดยเคร่งครัด รวมถึงการปฏิบัติตามนโยบาย ประกาศ คำสั่ง ระเบียบ และข้อบังคับของผู้ให้บริการ ตลอดจนกฎหมายที่เกี่ยวข้องกับการใช้บริการทั้งที่ใช้อยู่ในปัจจุบันและที่จะเกิดขึ้นในอนาคต อาทิ พระราชบัญญัติว่าด้วยการกระทำความผิดทางคอมพิวเตอร์ พ.ศ. 2560 , พระราชบัญญัติว่าด้วยธุรกรรมอิเล็กทรอนิกส์ พ.ศ. 2562 เป็นต้น และหรือกฎหมายอื่นที่เกี่ยวข้องตลอดจนถือเป็นการกระทำความผิดทางวินัยด้วยแล้วแต่กรณี
- การแก้ไข เปลี่ยนแปลง หรือปรับปรุงเงื่อนไขภายใต้ข้อตกลงการให้บริการนี้ ผู้ให้บริการจะแจ้งให้ผู้ใช้บริการทราบ ผ่านทางช่องทางต่าง ๆ ตามความเหมาะสมกับสถานการณ์
ผู้ใช้บริการได้อ่านและยินยอมให้ผูกพันตามเงื่อนไขภายใต้ข้อตกลงการให้บริการนี้

This agreement to identity verifies the person before using the prototype of the technology that helps to associate the health information of persons through high-speed digital networks.
“AirMed” is the prototype of the technology that helps to associate the subscriber’s health information through high-speed digital networks Services that persons can access the prototype of the technology help to associate the health information of person and airmen through high-speed digital networks and for the online consultations with nurse and physician. The subscriber can log in to the system, only once, and through the e-authentication service and access multiple systems within the project without having to log in again. Such services will be controlled and secured with appropriate measures as jointly determined by the Institute of Aviation Medicine and Naresuan University or scheduled in the future.
In which this service agreement provides
"Service Provider" means the Institute of Aviation Medicine and associates.
"Subscriber" means an applicant for access the prototype of the technology that helps to associate the health information of airmen through high-speed digital networks.
The service provider and the service user agree with the following statements.
- The subscriber must apply for the service or register through the channels specified by the service provider. The subscription will be free; however, the subscriber can use the service after the registration has been completed.
- The subscriber must provide personal information or details related to the subscriber in truth. For the benefit of using the service, If the service provider detects that the personal information or details related to the subscriber are inaccurate, inaccurate, or false. Whether the subscriber provides that information voluntarily or with negligence. The service provider has the right to cancel the subscriber's account without prior notice.
- In some cases, using the service, the subscriber must verify their identity using e-Mail or 13-digit national ID number as specified by the service provider.
- It is prohibited to impersonate or take any action which violates of privacy rights. By using other people's information in applying for services or registering through various channels, the offender must be legally punished.
- The subscriber give permission to the service provider to use information from service application or register through various channels to have access to the service.
- The service provider will keep the subscriber information confidential which will not disclose such information for commercial use or do any other action to legal proceedings with service subscribers giving permission.
- The subscriber must keep the username and password confidential. If it appears that someone else can use it through the subscriber 's account. The provider is not responsible for any damage incurred.
- The subscriber must strictly comply with the conditions under the service agreement. Including compliance with the policies, announcements, orders, regulations, and regulations of the service provider as well as laws relating to the use of services both currently in use and that will occur in the future, such as the Computer Crimes Act, B.E. 2017, the Electronic Transactions Act, B.E. 2019, etc. or other related laws as well as being considered an offense of discipline depending on the case, or both.
- Amendments, changes, or improvements to the terms under this service agreement. The service provider will notify the user through various channels as appropriate for the situation.
The subscriber has read and agreed to be bound by the conditions under this service agreement.
